About 500 years ago during the time of Chosun Dynasty, Korea boasted a rigidly hierarchical and male dominated social structure. Set in this period, "Jewel in the Palace" is based on a true story about a legendary girl (Jang Geum) who became the first woman to be the supreme royal physician of her times. Despite her poor condition as a low class girl in the male dominated society, Jang Geum overcame a series of social discrimination and landed herself as a royal cook, later becoming the royal physician, then ultimately the physician in charge of the king. She was even given by the king the title "The Great Jang Geum." The story of her checkered life on her success and breakdown as well as her love story beautifully unfold. Jung Ha Yoon is a graduate from law school who successfully passed the bar exam. In order to become a prosecutor specialised in the medical field malpractice litigations, she came to the Song Eui General Hospital as a 1st year intern. She works at a hospital to have a better perspectives to experience what the other side is all about and gain experience. At first she has no intention to be in the medical profession.

Tae Soo tries to steal from a gang. The gang catches Tae Soo's brother, Tae Jin, instead. The incident causes the brothers to separate. Tae Soo becomes a mob fixer and an assassin. Tae Jin becomes a police detective. When a call brings the brothers together, they get ready for brotherly bonding, but Tae Jin gets killed. Tae Soo decides to get revenge on his brother's killer.

Go Bok Su has had a difficult childhood and served time in prison for stealing. Jeon Kyung, a moody tomboy and a daughter of a rich family, plays the keyboard as a member of an unknown indie rock band. The vocalist of Kyung's band is diagnosed with a brain injury from an accident, and while the band members are trying to earn money for the surgery, Bok Su steals Kyung's wallet. Bok Su decides that their meeting was fate, leaves his long-time girlfriend Mi Rae, and follows Kyung around trying to win her heart. One day Bok Su finds out he has a terminal illness and doesn’t have much time left, and his life begins to change.

Seong Ri and Kyeong Bin are twins born via artificial insemination, later separated at birth. Both twins are unaware of each other until Seung Ri's mother abandons her one day and forces her to go to her real father. Seung Ri misses her mother and has difficulty in adjusting to her new family, including her twin brother and 3 sisters. Her friend's brother Soo Cheol befriends her eldest half sister who is able to hear but unable to speak due to an accident during childhood.
