What follows is the love story of Hong Shim, 'Joseon's oldest unmarried woman,' and Lee Yul, the amnesiac Crown Prince.

Gong Woo Jin (Yang Se Jong) is a 30-year-old single man who works as a set designer. Due to a trauma he experienced 13 years ago, he does not want to have a relationship with others. Meanwhile, When Woo Seo Ri (Shin Hye Sun) was 17, she fell into a coma. Thirteen years later, she wakes up from her coma. Her mental age is still that of a 17-year-old, but she is now 30-years-old. Gong Woo Jin and Woo Seo Ri then get involved with each other and fall in love.

In 1978, Eun-Joo, a child from a wealthy family, is kidnapped. Her kidnapper though doesn't make contact with the parents. Eun-Joo's mother and aunt then seek out various fortunetellers for help. The only fortuneteller to tell them that Eun-Joo is still alive is Kim Joong-San (Yu Hae-Jin). Kim Joong-San also recommends to the women that they get in touch with Detective Kong Gil-Yong (Kim Yun-Seok) for his help. Detective Kong Gil-Yong doesn't want to become involved the case, but he agrees to do so. He advises Eun-Joo's parents to keep the investigation private. Publicity could place Eun-Joo in more danger. Detective Kong Gil-Yong doesn't trust the fortuneteller, but he predicts things that come out to be true, including that the kidnapper will get in touch with the parents on the 15th day after her disappearance. With a picture that the kidnapper sends on the 15th day, Detective Kong Gil-Yong and Kim Joong-San are certain Eun-Joo is still alive.

Jong-Dae (Lee Min-Ho) and Yong-Ki (Kim Rae-Won) grew up in an orphanage like real brothers. After they leave the orphanage, they make a living by picking up paper and empty bottles. Jong-Dae then becomes involved in a struggle of interests over development in Gangnam, Seoul. In the process, he lives with Gil-Soo (Jung Jin-Young) and Sun-Hye (Seol Hyun) as they accept Jong-Dae like a family member. Jong-Dae also becomes a gang member.

Sung-Hoon (Kim Woo-Bin) was raised by his mother and doesn't have any memories of his real father. His childhood was miserable, surrounded by his mother's gangster friends. He eventually became a gang member himself. He is now imprisoned for fighting against other gang members in a Buddhist temple. Meanwhile, Joon-Seok (Yu Oh-Seong) has been in prison for almost 17 years and is set for release in the near future. A woman comes to the prison and requests to talk with Joon-Seok. The woman was in his circle of friends during his high school days. She informs Joon-Seok that her son Sung-Hoon is an inmate in the same prison. She asks Joon-Seok to protect her son from other gang members. Joon-Seok soon takes Sung-Hoon under his wings in prison. When Joon-Seok is released from prison he learns that Eun-Ki (Jeong Ho-Bin) has gotten rid of his men and now dominates the gang which Joon-Seok's father (Joo Jin-Mo) started. Joon-Seok plots to take back control of his gang. Meanwhile, Sung-Hoon is released from prison and works for Joon-Seok in his power struggle against Eun-Ki. 'Gu Am Heo Joon' is a remake of the 1999 drama Huh Joon about the dramatic life of the writer of the eastern medicine manual (Dong Eui Bo Gam), Heo Joon. It depicts the life story about a commoner who rose up the ranks to become a royal court physician.

The drama depicts the difficult decisions and moving human stories that take place daily in the OB-GYN department, focusing in particular on Seo Hye Young, a talented and determined obstetrician who has just transferred from the prestigious Seoul base to the branch hospital. This novel-based story revolves mainly around four people who are drawn to Seogwipo, Jeju for a project. Kim Jung Woo and Lee Soo Hyun first met when they were children but lost track of each other for numerous years. Years later when Jung Woo meets Soo Hyun, he falls in love with her but his feelings are not reciprocated. Soo Hyun doesn't realize until much later that she's drawn to Jung Woo as well.

In school, she meets the notorious F4, the most popular and powerful group of boys at the school, consisting of Gu Jun Pyo; the leader of F4 and heir to the Shinhwa Group, Yoon Ji Hu; the grandson of a former president of Korea, So Yi Jung; a skilled potter who comes from a family that owns the country's biggest art museum, and Song Woo Bin; whose family runs the country's largest construction company.

Her life at school starts out miserable, as she doesn't fit in with other students because of her status, and later becomes worse when she is labeled as the new bullying target of the F4.

Jumong was the founder of Goguryeo in 37 BC. He was the son of General Hae Mo Su and Lady Yoo Hwa, and was raised by King Geum Wa who took him and his mother in when Hae Mo Su was believed to have been killed in an ambush by the Han Dynasty. So Seo No was a merchant's daughter who helped Jumong in realizing his dream to build a new country, and later became his second wife.

Mi Ju is a part-time music professor with a tragic past. When she was younger, she was an up-and-coming cellist until she was in a car accident with her rival/best friend, Tae Yeon. After Tae Yeon died, Mi Ju gave up playing the cello and decided just to teach. One day, one of her students threatens her after receiving a bad grade, and so begin Mi Ju's problems. She is sent a mysterious cassette tape that contains a haunting cello duet that dredges up some painful memories of Tae Yeon and almost causes her to get into a car accident.

She is distant from her loving family, which includes her two daughters, husband and sister-in-law. Her older daughter is mentally disabled and mute but falls in love with a cello she spies in a window. Mi Ju buys it for her and begins to teach her how to play it. Meanwhile, a new maid moves into the house. She is creepy as hell and is also mute. It seems as though her family was killed in a car crash, and she tried to commit suicide numerous times, once by swallowing acid. No more vocal cords there!

Anyways, things start to get creepy slowly but surely. The family's golden lab, Sunny, begins barking all the time until one morning he is found dead. Mi Ju starts seeing things, and her daughter begins playing that haunting song on the cello. The sister-in-law goes crazy after her fiance breaks up with her, and "accidents" befall the household. Is Tae Yeon out for revenge, or is Mi Ju's guilt driving her crazy?

The story is narrated by the younger brother of a cleft-lipped boy, who is the runt of the family and thus gets all the attention from his doting mother. The boys' father left them because of his son's disfigurement, and shortly after that, the mother discovered she was pregnant (with the narrator).

The father leaves little behind - except an old camera. The show begins and ends with that camera. As the boys grow up, there is rivalry for a girl, and the older son's heart is broken as his brother plagiarises his love poetry for her, and ends up going out with her instead. Still, the older son shows love and patience to his brother, despite getting constantly abused by him. Eventually they start to get along. But there is one thing the narrator is unable to do - call his disfigured sibling ‘his brother'. His whole life has been filled with jealousy towards his older brother, and resentment towards his mother for the unequal treatment.

While the older son goes on to medical school, the narrator falls out with society, his mother, and even his brother, becoming a debt-collector. Ultimately a string of unfortunate misdeeds which he commits, comes back to haunt him, and it is finally then that he can call his cleft-lipped sibling ‘his brother'.
